The following form must be completed and handed to the Volunteer Manager during the in-person interview.\n\t\t<\/p>\n\t<\/div>\n\t<p><b>Parental Consent Form<\/b>\n\t<\/p>\n\t<p>I hereby consent to the participation of my daughter\/son, in the KACCH & BACCH Volunteer Programme.
