    يجب على احدى والديك أو الوصي عليك تعبئة نسخة ورقية من هذا النموذج إذا كان عمرك أقل من 18 عامًا. يجب إستكمال النموذج التالي وتسليمه إلى مدير برنامج التطوع أثناء المقابلة الشخصي.Your parent or guardian must complete a hard-copy of this form if you are under 18 years old. The following form must be completed and handed to the Volunteer Manager during the in-person interview.

    أوافق بموجبه على مشاركة ابنتي / ابني في برنامج كاتش وبيت عبدالله التطوعي. أني على علم بأن ابني / ابنتي يجب أن يفي بمتطلبات البرنامج التطوعي من أجل الحصول على شهادة لمشاركته التطوعي.I hereby consent to the participation of my daughter/son, in the KACCH & BACCH Volunteer Programme. I understand that my son/daughter must fulfill the volunteer programme requirements in order to receive a certificate for his/her volunteer participation.

    كما أنني أعطي الإذن باستخدام أي صور تم التقاطها لابنتي / ابني أثناء تطوعه مع كاتش وبيت عبداللهللاستخدام بغرض الدعاية و الترويج لخدمات كاتش وبيت عبدالله دون قيود أو تحفظ. I also give permission to use any photographs that are taken of my daughter/son, while s/he is volunteering with KACCH & BACCH, for the use of publicity in promoting KACCH & BACCH activities without limitation or reservation.
